The
goal of MSSP is to help seniors continue living

senior is assigned a nurse and social worker who
assess needs and provide ongoing care management. |

OPED
offers paratransit services to residents of Oakland
and Piedmont who cannot access public transportation
and are age 70 and older, or 18 and older with
a mobility disability. Taxis and wheelchair-lift
vans provide transit to medical appointments, shopping
and other activities. |

Senior
Companions make a difference by being a friend
to other older adults, and helping with grocery
shopping and other daily tasks. Foster Grandparents
help at-risk children and youth by giving them
much needed attention, advice on life issues, and
help with their studies. Volunteers who are income-eligible
receive small stipends and community recognition
for their dedication and service. |

Linkages
connects disabled adults (18 years and older) and
frail older adults to community-based programs,
enabling them to remain living in the community
rather than in nursing homes. Each client is assigned a social worker who assesses needs and provides care management. In addition, information and referral
services are provided to clients as well as other other
residents of the East Bay. |
